If you were a patient on or before May 03, 2022, and received substance abuse treatment from one of the following providers at The Center for Emotional Care, Dr. Mukesh Patel, Dr. Himanshu Patel, and Dr. Jagdeep Wander, you have the right to seek revocation or amendment of a Court Order authorizing the disclosure and use of your substance abuse treatment records issued by the United States District Court, Western District of Virginia (Title 42 United States Code, Section 290dd-2 and Title 42 Code of Federal Regulations, Section 2.66). If you want to exercise your right to revoke or amend the Court Order in relation to the use or disclosure of your patient file, or if you want a copy of the Court Order, you may contact the U.S. Attorney’s Office for the Western District of Virginia at 540-857-2250.
